Your insurance card is required for Medicare (Part B), Anthem Senior Advantage, SummaCare, or
Hometown Health. Persons on Medicaid, WellCare, CareSource, and Buckeye your card is also required.
Please bring a driver’s license or state issued ID card.
In county flu shots are $25 and in county pneumonia shots are $35.
Another service provided by your Health Levy.
